PRK introduced in 1987, is a predecessor of Lasik. The success of the procedure can be determined from the fact that even after 27 years, this treatment remains as the primary option to treat many cases of refractive error. While advance Lasik has fast replaced almost all other refractive surgeries, PRK is still as successful as it was the time it was launched. Patients with large pupils or thin corneas have been treated successfully with PRK.

Photorefractive Keratectomy, or PRK, like other refractive procedure, brings about a change in the shape of the cornea in-order to treat the underlying aberration. PRK and its successors (Lasik) thus not only share the same underlying principle to treat cases of near sightedness, farsightedness and astigmatism, but also utilize the same excimer laser to ablate the anterior central cornea and change its shape in the process.

While PRK and Lasik, both aim at changing the corneal shape to bring about the correction required, the manner in which they go about it is what defines each of these processes. Patient selection for these procedures, post procedure recovery time and also the possible risks and side effects associated with each of these procedures is case specific and shows interpersonal variances.

PRK does not create a corneal flap, a step that forms the basis of present day Lasik treatments. Instead, it completely removes the superficial layer of the cornea (epithelium) to expose the underlying deeper corneal layers, the site where actual ablation takes place. The epithelium is a rapidly growing, soft layer in contact with the tear film. New stem cells (form the limbic system) quickly migrate to the site of injury and in matter of days; a new corneal epithelium is thus created.

The banes of this technique are numerous and people who have persistent dry eyes, thin corneas and large pupils are excellent candidates for this procedure. Since only 5 -10% of the cornea is removed during the procedure, the corneal strength and stability is not compromised, furthermore, the amount of cornea left for actual ablation to take place is sufficient enough so as to spare the deeper stromal layers. Absence of a corneal flap effectively nullifies any flap complications that might accompany a regular Lasik procedure.

As, with any surgical procedure, there are some disadvantages associated with PRK as well. These largely relate to the technique of PRK where the corneal epithelium is removed in totality. The main disadvantage is the postoperative discomfort and speed of recovery.

With PRK, the rate of recovery is slow because the removed epithelium will take few days to regrow, however, the quality of healing is excellent. For the next 6 months your eye will continue improving dramatically and within 6 weeks to 3 months, you will achieve your best-corrected vision.

Using antibiotic eye drops reduces the risk of developing infection and mild steroid eye drops help in reducing inflammation and any discomfort (irritation or foreign body sensation) you might experience. After the surgery, your doctor will place a bandage contact lens in your eyes to aid in healing and minimize the discomfort. After 3 to 5 days, this bandage is removed and even though your eyes will be sensitive to light, you can manage well by wearing dark protective eyewear. You must avoid driving for 1 week after the procedure and practice caution for the next 6 months so as to avoid any unnecessary trauma.

For the next 6 months however, you must meet your doctor regularly to assess recovery at every stage. Even though complete healing takes time, every passing day, your vision improves and you will able to appreciate the new improved vision in matter of few weeks.
